Overview:

Under the direction of the Clinical Manager and Director of home care services, the RN/LPN identifies the needs of assigned patients and delivers skilled nursing care in the patient's home. The RN/LPN performs assessments, plans, and delivers patient care to meet the patient's needs according to physician's orders. The RN participates in operational aspects of the department and participates in quality improvement activities. Participates in all infection control, departmental equipment training, organizational safety and fire safety programs.

Monday-Friday - 8am-4:30pm

Call is required

Qualifications:

Experience: Graduate of an accredited school of nursing

Required Education: Nursing Degree from an accredited school

Preferred Education: BSN

License: current RN or LPN license in our state

Certifications: Basic Life Support certification through the American Heart Association
