Under the direction of the Director of Population Health, the Population Health Coordinator partners with patients, providers, and other members of the clinical care team to implement standard work processes that drive improvement in quality and patient outcomes. This role will apply value-based care standards and utilize standard analytics tools to drive performance.
This role will also work in conjunction with local and Ministry Population Health teams to ensure goal alignment, identify opportunities for improvement and provide feedback on progress.
The Population Health Coordinator will perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Experience: 3+ years clinical healthcare experience
Required Education: High school diploma
Preferred Experience: Medical assistant certification, background in education or social work, Knowledge of ambulatory clinic operations and HEDIS quality measures
Ability to connect with patients and break down barriers to receiving appropriate care
Excellent written and oral communication skills
Excellent organizational, interpersonal, and analytical skills
Attention to detail and accuracy
Proficiency in MS Office (Word, Excel, and PowerPoint
Self-directed and able to perform independently with minimal oversight
