Job Duties would include, but not limited too:
+ Provides care and education to patients and their caregivers
+ Participates in the coordination of services with other disciplines and community resources as needed
+ Evaluates and revises care plans/assessments based on changes in patients and/or their environments
+ Participates in regular case conferences and in-services or special case orientations as needed and participates in on-call rotation

+ An active LPN license in the state of Arkansas
+ Graduate of an accredited school of Nursing
+ One year of LPN in a home health setting
+ Proof of eligibility to pass medications required
+ The ability to think and act quickly and discern the difference between urgent vs. emergency situations
+ Passion for excellence in quality of care
+ Must have good communication skills
+ Must have good interpersonal skills
